Your First Visit

Patient Forms

Upon entering our office, our receptionist will welcome you as a member of our family. If not already done, we will request that you complete our patient forms located here online in the New Patient Center. This paperwork provides us with your health history and information on your condition so we can begin to determine if we can help.


Consultation

Next, you will have a consultation with your doctor to discuss your health-related problems, concerns, and potential treatment options. This initial visit is designed for us to learn more about you, your condition, and expectations to decide if our care is the best way to meet your goals.


Examination

After your consultation, we will perform an examination based on your sings and symptoms. However, nothing will be done in our office without your consent.


X-Ray Studies

We do not routinely take X-rays. If you have previous X-rays or other imaging studies, we may request these records. If your symptoms and our tests suggest imaging studies are needed, we will refer you for them.


Report of Findings

Once the information is collected and examinations are performed, we will give you a detailed report of all findings and answer any questions including:

  1. How can you help me?
  2. How often do I need to come in?
  3. What will my treatment cost?

After reviewing your health history, goals, and examination findings, presuming that we find our care is indicated in your case, we will discuss our recommendations. We will also notify you if your condition requires care with other providers.


Treatment

At the conclusion of this initial appointment, you can choose to experience your first treatment. This may include spinal adjustments, physical therapy, laser and/or soft tissue massage. For those receiving Nutritional Response Testing and Designed Clinical Nutritional or other holistic therapies like Aqua Chi Detox , Bach Flower or Bio-Meridian Health Analysis, the initial natural therapy or supplemental herbal and/or homeopathic prescription will commence. Treatment is interactive, so you can express concerns about the different styles of treatment anytime.


Activities of Daily Living (ADLs)

Prior to leaving, and/ or on subsequent visits, your doctor may suggest ADLs to incorporate outside of treatment room. These may include ice or heat application instructions, certain activities or positions to avoid, natural pain relievers, orthopedic supports or at home exercises and/or stretches. Every person is unique, therefore everyone requires a customized plan. The purpose of our total program is for you to get well, be well and stay well through a personalized holistic plan that fits your goals and lifestyle.
